2018 FLAG RUN SPONSOR FORM
This form is to be used for anyone wishing to donate raffle items, register as a vendor, or sponsor the event.

SPONSORS: This event has various needs and expenses such as banners, insurance, promotional items, etc.
If you wish to provide any items just list them, or if you would like to send us a donation to help, PayPal it to kyle@desertfanatics.com
This donation will not go to the charity, but will be used 100% towards the event. Please fill out the form if you have sent a donation and note the PayPal email.

VENDORS: You must have a Glendale, AZ TPT if you wish to sell merchandise.
Vendor fees: $100 or SINGLE raffle item of at least same value. (We will send you information to send fee directly to the charity.
501(c)(3) tax information can be sent for raffle donations and fees. Please request this in the description box.)
Filling this out ensures that we will keep you updated on vendor info.

NOTICE: Due to the large amount of items, small donations such as clothing items, koozies, stickers, etc. are very welcomed but will not be entered into the raffle. They will be bundled and given to donating entrants of the Flag Run.

IMPORTANT: IF YOU DO NOT RECEIVE YOUR COPY OF THE FORM, PLEASE CHECK YOUR SPAM AND MARK THE EMAIL AS NOT SPAM. YOU WILL NOT RECEIVE YOUR RESPONSE FROM US IF YOU DO NOT DO THIS.

Submitting this form does not guarantee your spot as a vendor. Please await contact from us. In some cases Westgate will be the approving party. Please no food vendors or businesses in direct competition with a resident of Westgate.
